[Dcmosls] Professional Development Evaluation forms, Liaison meeting 10/19

Hello;
 
To all who were able to attend the SLS library liaison workshop/meeting of 10/19, we thank you, and encourage your sustained feedback and evaluation. 
 
Attached are the evaluation form for all workshops. This form is new.
I am also including the Categories list from which to populate the first column.
Technology, 11, and Technology Skill, 12, will be obvious choices, but I feel there may be others as well.
 
Please fill out the form to the best of your ability, if you were in attendance, and return, either in paper or electronically, the completed reflection/action steps, to Rebecca Dixson. In two weeks we will send out the follow-up impact survey.
