Hello;
Survey Monkey has a highly customizable survey tool. The 'free' version limits the user to 10 questions per survey, although you can send it to unlimited users. It does analyses and tabulation of the results.
The full version is only $20 per month, and you have the option of turning it off in months you do not use it - though your data is stored but inaccessible. This version can be re-logo-ed with school emblems, colors, etc - no one even knows it is survey monkey.
I would be happy to work with you committee and help draft the survey using these tools, if help is desired. Sidney is doing a lot with i-Safe, as is Afton.

Good Morning,
Our Technology Committee will soon be surveying our students asking
questions such as: Do you have a computer at home?, Where is it located,
Does it have Internet access, Do you have an IM name or use IM, Do you
have a MySpace account, etc.
We are doing this in conjunction with the Elementary ISafe Curriculum.
Has anyone else done a similar survey and/or have any suggestion about
how to put one together?
Any examples or help would be appreciated. Thanks
