[Dcmosls] SLS Member Plan Form Reminder

Hello Everyone,
 
There is a new SLS Member Plan available that can be filled out using Word and sent as an email attachment.
If you could please fill out the SLS Member Plan before the end of school, it would be most appreciated. 
The established cut-off date is June 15, 2007.
 
Instructions:
Copy and paste the 'SLS Member.doc' attachment in this email to your desktop.
Open the 'SLS Member.doc' document that you just saved to the desktop and fill out the first 9 pages and save.
The Agreement page (last page) is to be printed and signed by the administrators, but can be mailed in at a later date(instructions to print are below).
 
After filling out your SLS Member Plan please email as an attachment to Ronald Pike at piker at dcmoboces.com. 
 
To Print the Agreement Page(last page):
In Word go to the main menu select  File > Print > under Page Range select the 'Page' button and in the box to the right enter 10.
Then select 'OK' to print (this setup will print just page 10).
 
Online Version:
The SLS Member Plan is also available online at http://www.dcmoboces.com/dcmosls1/pos.htm.
Select 'Member Plan 2006-2011(Word Document - Form Version)' from the menu which will open up as a Word document.
Select 'File' > 'Save as' to save to you desktop.
