[Dcmosls] SLS Annual review form, please fill out and return

Attached is the review form for the School Library System 2006-2007.
This should have been mailed in June, but was not.
This was a rookie error - I apologize. 
 
If you could take a moment to fill it out and return it before September 25, so that it may be reviewed at our September 26 SLS Council meeting, this is greatly appreciated.
 
It is a form-fillable document. Save it to your desktop, fill in all the grey active fields, save, and return as an email attachment.
Of course you may also print and fill in by hand. The electronic option is preferred.
